Procedure for issue of replacement certificates, transcripts or diploma supplements

 

This procedure will be used when:

a) An alumnus requests an new copy of a College or University certificate; or

b) A new copy of an academic transcript or diploma supplement.

College Certificates

A new College certificate can be produced on request, upon receipt of a signed statement accompanied by a print or scan of the photo page of your passport, or scan or photocopy of your photo driver’s licence.  The cost for a replacement certificate is £25. 

Provision of the following information will enable the College to process your request in a timely fashion:

• Your student number;

• Your full name (the name used when you were studying);

• Your date of birth; • 

The course you studied; and

• The years of study.

University degree certificates

Universities will not normally issue additional copies of a degree certificate; there can only be one in existence.  They may however be able to provide a certified copy, or a replacement.

If you have damaged your certificate, your University may replace it free of charge, on receipt of the damaged original. 

A charge is levied for replacing lost certificates, which is currently:

• £30 for the University of Wales

• £35 for UCLAN; and

• £45 for Middlesex University.

Requirements for proof of identity vary, but it is likely you will be asked to provide a signed statement confirming the original has been either lost or destroyed, and/or:

• Your student number;

• Your full name (the name used when you were studying);

• Your date of birth;

• The course you studied; and

• The years of study.

Contact the relevant University via their website – www.wales.ac.uk, www.uclan.ac.uk or www.mdx.ac.uk and search for ‘replacement certificate’.

Transcripts or diploma supplements

Transcripts and Diploma Supplements are produced according to guidelines arising out of the Bologna Process, an intergovernmental initiative designed to create a European Higher Education Area (EHEA) by 2010 and to promote the European system of higher education worldwide. 

Strict guidelines are set out regarding what is included in the document. It is not possible for students to request additional information to be included on their Diploma Supplement or transcript.

Under the Bologna process, the College can provide a replacement transcript or diploma supplement for students who obtained their diploma before programmes were validated by a university. 

This will produced upon receipt of a signed statement accompanied by a print or scan of the photo page of your passport, or scan or photocopy of your photo driver’s licence. 

The cost for a replacement academic transcript/diploma supplement is £10. 

Provision of the following information will enable the College to process your request in a timely fashion:

• Your student number;

• Your full name (the name used when you were studying);

• Your date of birth;

• The course you studied; and

• The years of study.

Your University may be able to provide you with a diploma supplement or transcript. 

Middlesex University will provide a supplement at a cost of £10. 

You will need to contact the University of Wales or UCLAN direct for their costs. 

You may need to provide them with the same information as for a certificate:

• Your student number;

• Your full name (the name used when you were studying);

• Your date of birth;

• The course you studied; and

• The years of study.
